How do I sign up for a MyCARE Info portal account? If you know your Medical Record Number and provided your email address when you last registered, you can go to and click on the link next to ‘Need an Account?’ If you do not know your Medical Record Number then you can do one of the following:
  1. Complete the Request for Access form on the page, supplying as much information as possible. Once the form has been received and processed, depending on how much information you provided, you will either receive an email with a one-time username and password or a call from our Medical Records Department. Please allow 48-72 hours for your request to be processed.
  2. Visit the Medical Records Department at either Roger Williams Medical Center or Our Lady of Fatima Hospital and a staff member will enroll you in the portal. You will then receive an e-mail with your one time username and password and instructions.
I don’t have an email address, do I need one for the MyCARE Info portal? Yes, an email address is required to obtain access to MyCARE Info. If you do not have one, you may create an email account free of charge from various sources online. Some email providers are listed below: Is there an age requirement to have a MyCARE Info account? Yes, individuals must be at least 18 years old to create an account with MyCARE Info. Can I see my family members’ health records through MyCARE Info? Yes, this is called Proxy Access. Proxy Access requires that a Release of Information form be filled out in person with the Medical Record Department at either Roger Williams Medical Center or Our Lady of Fatima Hospital. What information can I see in MyCARE Info? You can view information from your CharterCARE Health Partners’ Electronic Health Record including some test results, physician reports, home medications, allergies, discharge instructions, upcoming and past appointments. You may also print, download, or send your Health Summary. **Please note, Roger Williams Medical Associates Offices (CCHP Physicians, LLC) or Elmhurst Extended Care information will not be included in MyCARE Info. Is MyCARE Info secure? Yes, your health record is secure on the MyCARE Info portal. Access to MyCARE Info is controlled through personal Logon IDs and encrypted Passwords. URLs are also rewritten so that they cannot be copied and pasted. Only you, authorized family members or healthcare proxies (people YOU designate) will have access to your records via MyCARE Info. To further protect your information, do not share your MyCARE Info logon ID or password with another person. What if I need more information than is available through MyCARE Info? If you need additional information or a more complete record, please contact our Medical Records department at 401-456-2180. I forgot my password. What should I do? Click the ‘Forgot Password?’ link located on the log-in page. A temporary password will be emailed to you. Click on the link and follow the directions to reset your password. If you’ve forgotten your username, you will need to contact us at (401) 456-2555 to have it reset. How can I reset my password? After you enroll in MyCARE Info, you will use your one-time username and password and then choose your own username and password. When you are logged into MyCARE Info, you can change your password anytime by selecting ‘Preferences’ at the bottom of the home page, and then the ‘Change Password’ button on the right. How can I change the e-mail address where MyCARE Info notifications go? You may also change this under the Preferences link at the bottom of the home page. What if I have problems or need help with MyCARE Info? If you have any questions regarding the MyCARE Info portal, you can email us at or you can call (401)456-2555. As the phone number will be staffed during normal business hours (9am to 5pm M-F), please allow 48-72 hours for a response. If some of my health information on the MyCARE Info patient portal is not accurate, what should I do? Your MyCARE Info health information comes directly from your electronic health record with CharterCARE Health Partners. You may either print an Amendment Request form & submit it to the Medical Records department at your facility (either Roger Williams Medical Center or Our Lady of Fatima Hospital) OR you may inform a clinician during your next hospital visit.
